This Privacy Policy describes how Zack Kampf, PhD, LCSW (“I,” “me,” or “my practice”) collects, uses, and protects information submitted through this website.

Information I Collect

When you submit the contact form on this website, I collect the information you provide, which may include your name, email address, phone number, and any details you choose to share about what brings you here. This website also collects standard analytics information (such as pages visited and general location data) through Google Analytics.

Important note about the contact form: This form is intended for general inquiries and scheduling a free consultation. It is not a secure clinical communication channel, and it is not monitored in real time. Please avoid including highly sensitive details in your message beyond what’s needed to schedule a consultation. Once we begin a clinical relationship, all communication will move to HIPAA-compliant channels appropriate for protected health information.

How I Use This Information

Information submitted through the contact form is used solely to respond to your inquiry and schedule a consultation. I do not sell, rent, or share your information with third parties for marketing purposes. Your information may be stored in Squarespace’s form submission system and, once we begin working together, in a HIPAA-compliant electronic health record system.

Cookies and Analytics

This website uses Google Analytics to understand general site traffic and usage patterns. This tool may use cookies to collect anonymized data about how visitors use the site. You can opt out of Google Analytics tracking through your browser settings or available browser extensions.

Your Rights Under California Law

If you are a California resident, you have certain rights under the California Consumer Privacy Act (CCPA), including the right to know what personal information is collected about you, the right to request deletion of your information, and the right to opt out of the sale of personal information. I do not sell personal information. To exercise any of these rights, contact me at zack@zackkampf.com.

I take reasonable measures to protect the information submitted through this website. However, no method of electronic transmission or storage is completely secure, and I cannot guarantee absolute security of information submitted online.
